How to Enroll

- In order to enroll in a class, you must pre-register first.
- To pre-register please complete following:
1.
Complete a class registration form for the class you wish to
enroll in.
2. Obtain a copy of your pet's vaccination record showing proof of the required vaccinations and staple it to your registration form. With your permission we are happy to call your veterinarian for you to obtain the proof of vaccination if you would like.
3. Submit the registration form, a copy of the pet's vaccination record, along with payment to Blair Animal Clinic.
- Registrations may also be completed by mail or fax, keeping in mind it may cause a delay in the enrollment process.
- Registrations submitted by phone or fax must be paid by credit card.
- We accept Mastercard, Visa, Discover, personal checks, or cash.
- Once we recieve your registration and payment, you will recieve a confirmation by e-mail. (Telephone confirmation must be specified during registration and is used only if you do not have a current e-mail address).
